To receive you Bausch & Lomb rebate check by mail, follow these 5 steps:
1. Get an eye exam
2. Purchase PureVision contact lenses from January 1, 2009-December 31, 2009. Product purchase must be made within 90 days of eye exam. Circle the eligible purchase on your receipt or invoice.
3. Visit www.bausch.com/rebates and input this single-use Personal Code to complete the offer from online. Limit 1 per personal code. Complete all information requested online accurately.
4. Remove the original UPC codes from the PureVision boxes (4 UPC codes for the $50 rebate, 2 UPC codes for the $20 rebate.)
5. Mail a copy of your receipt(s) for eye exam and product purchase, original UPC codes, and completed offer form to the address provided on the offer form. Submission must be postmarked within 60 days of purchase.
